These are kept written, electronically and confidential. These records are kept so that the individual can be monitored how their dementia is progressing. This gives a timeline of when each sign or symptom changes. Changes in memory, ability to do daily activities, behaviour, and personality should all be recorded. We also record if an individual has refused their daily medication, how much each individual eats as well as each day writing about the individuals mood and how much support that the individual has needed with personal care. Any changes to the individual can show progression to their dementia. We are then able to change the individual’s care plan to suit the changing needs of the individual.
